The single biggest mistake is leaving a meaningful amount of crypto sitting on an exchange long term, treating it like a bank account rather than understanding that exchange custody means someone else genuinely controls the actual keys. If that exchange gets hacked, goes insolvent, or freezes withdrawals, funds held there are genuinely at risk in a way self custody wallets simply aren't.

A closely related mistake is moving to self custody without actually understanding the responsibility that comes with it. Since a lost seed phrase with no backup means permanently lost funds with genuinely no customer support line to call and no way to recover it, unlike a forgotten bank password that can usually be reset.

People also frequently store their seed phrase digitally. A photo on a phone, a note in cloud storage, a screenshot, which defeats much of the actual security benefit, since that device or account then becomes a single point of failure that a hacker only needs to compromise once to access everything.

Another common trap is not testing a wallet recovery process before actually relying on it. Assuming a backup works without ever verifying it, then discovering during an actual emergency that a seed phrase was transcribed incorrectly or a backup device was never properly configured in the first place.

The honest summary is that most crypto losses trace back not to some sophisticated attack but to basic custody mistakes. Understanding the tradeoff between exchange convenience and self custody responsibility, and actually verifying backups work before you genuinely need them, solves the majority of what actually goes wrong
